主要对毛细胞白血病疗效很好,对其他淋巴瘤及白血病亦有效。
生物活性Cladribine(Leustatin, 2-CdA, 2-氯脱氧腺苷, 2-Chloro-2′-脱氧腺苷, CldAdo)是一种腺苷脱氨酶抑制剂,作用于U266、RPMI8226和MM1.S细胞,IC50分别约为2.43 μM、0.75 μM和0.18 μM。

Cladribine对毛细胞白血病(HCL)发挥显著的活性,延长完全缓解期。该药物通过诱导DNA链断裂累积并激活淋巴细胞中的抑癌基因p53来发挥作用。Cladribine可能调节MM细胞中STAT3活性。Cladribine在不同细胞系中表现出剂量依赖性的抑制作用:U266对Cladribine最不敏感,而MM1.S最敏感。Cladribine治疗导致细胞周期G1期的比例逐渐增加,并减少S期细胞数量。在某些条件下,Cladribine还可能增加U266细胞中G2-M期的比例。此外,在RPMI8226和MM1.S细胞中观察到Cladribine剂量依赖性诱导的细胞凋亡。0.2 μM Cladribine治疗时间依赖性显著诱导MM1.S中的caspase-3、-8和-9激活及PARP裂解。Cladribine以剂量依赖的方式显著降低磷酸化STAT3 (p-STAT3)水平,但不影响总STAT3蛋白质水平。在HSB2细胞中,Cladribine具有浓度依赖性诱导细胞凋亡的潜能。Cladribine还以剂量依赖的方式抑制原代肥大细胞(MC)及其系HMC-1生长,并且含有KIT D816V突变的HMC-1.2细胞比无该突变的HMC-1.1细胞对Cladribine更敏感。Cladribine还降低了CD14表达。
体内研究Cladribine (0.7-3.5 mM)和/或diltiazem (2.4 mM),通过腹腔注射到成年斑马鱼,可分析心血管健康的潜在标志物——红血球(RBC)裂解物嘌呤核苷酸水平。Diltiazem能增强Cladribine的作用效果。
用途Cladribine是一种耐腺苷脱氨酶的脱氧腺苷类似物,主要用于治疗活动性毛细胞白血病。
生产方法2,6-二氯嘌呤(I)(0.95g, 5mmol)和氢化钠(50%分散油, 0.25g, 5.2mmol)悬浮于35ml无水乙腈,在室温及氮气保护下搅拌30min。在搅拌下20min内分批加入1-氯-2-去氧-3,5-二-O-对甲苯甲酰基-α-D-赤式-呋喃戊糖(II)(1.95g,5mmol)。加毕,再搅拌15h。滤去少量不溶物,浓缩剩余的油状物,并经硅胶柱层析,用甲苯-丙酮(9:1, 体积比)洗脱。得到的固体再用乙醇结晶,得1.60g化合物(Ⅲ),熔点159-162℃。
化合物(Ⅲ)(2.50g, 4.6mmol)溶于60ml饱和氨的甲醇溶液(0℃饱和),在100℃搅拌5h。蒸发至干,剩余物经硅胶柱层析,用氯仿-甲醇(8:2, 体积比)洗脱。再乙醇结晶,得0.87g克拉屈滨,熔点220℃(软化)。
