硝苯地平又称心痛定、利心平等,是一种二氢吡啶类钙拮抗剂。它能抑制心肌和血管平滑肌对Ca2+的摄取,扩张冠状动脉,增加冠状动脉血流量,提高心肌对缺血的耐受性。同时,硝苯地平还能扩张周围小动脉,降低外周血管阻力,缓解冠状动脉痉挛,增加冠脉流量,改善心肌缺氧,从而有效降低血压。在小剂量扩张冠状动脉时,并不影响血压,是较好的抗心绞痛药。
硝苯地平用于预防和治疗冠心病心绞痛,对呼吸功能无不良影响,尤其适用于冠状动脉痉挛所致的心绞痛及患有呼吸道阻塞性疾病的心绞痛患者。其疗效优于β受体阻滞剂,并且也适用于各种类型的高血压,包括重度及顽固性高血压。此外,治疗顽固性充血性心力衰竭时可长期服用本品。
硝苯地平还用于原发性肺动脉高压、弥漫性食管痉挛及支气管哮喘、十二指肠球部溃疡、尿道梗阻、运动性哮喘、贲门失弛缓症等疾病的治疗。它对血管平滑肌具有一定的选择性,对心脏的直接负性肌力作用及变性作用较弱,故全身给药不引起心率减慢而表现为心率反射性增加。
生物活性硝苯地平(BAY-a-1040)是一种二氢吡啶类钙离子通道阻滞剂,具有有效的血管扩张作用和钙拮抗作用。在人冠状动脉内皮细胞中,它显著浓度依赖性地增加了eNOS蛋白的表达。

体外研究在培养的人冠状动脉内皮细胞中,硝苯地平以浓度依赖性的方式增加eNOS蛋白的表达。硝苯地平拮抗整个心血管系统中的L-型钙离子通道,并阻断Kv通道(这是相同的超基因家族成员)。在血管平滑肌细胞(VSMC)中,硝苯地平以剂量依赖性方式降低[(3)H]-胸苷掺入、总细胞蛋白含量以及磷酸化的细胞外信号调节蛋白激酶(ERK)1/2、丝裂原活化蛋白激酶激酶的水平(MEK)1/2和Pyk2的磷酸化。在VSMC和球囊损伤胸主动脉血管VSMC中,硝苯地平剂量依赖性抑制增殖细胞核抗原(PCNA)的水平。
体内研究在大鼠中,硝苯地平(3 毫克/千克)略微降低收缩和/或舒张期血压的水平或增加心脏速率。在脉络膜小动脉平滑肌中,硝苯地平(1 μM)产生存储操作途径的最大抑制。在大鼠受伤粘膜中,硝苯地平(20和40 毫克/千克)显著阻止盐酸加乙醇诱导的胃粘膜损伤并增加的硫代巴比妥酸反应性物质水平。在大鼠溃疡粘膜中,硝苯地平(20和40 毫克/千克)剂量依赖性促进溃疡愈合并抑制硫代巴比妥酸反应性物质的增加。
化学性质黄色结晶状固体。熔点为172-174℃。易溶于丙酮、氯仿、乙酸乙酯,溶于热甲醇,不溶于水。遇光易变质。
用途硝苯地平是一种长效冠脉扩张药,能增加冠脉血流量,减少心肌耗氧量。适用于急慢性冠脉功能不全,特别是心绞痛及心肌梗塞的治疗。
生产方法粗品用甲醇重结晶即得成品,收率50%。
