制备方法与用途

呋喃硫胺简介

呋喃硫胺为白色或微黄色结晶性粉末，熔点在130-136℃（分解）。它易溶于甲醇、乙醇和氯仿，微溶于丙酮，不溶于苯与乙醚，具有轻微蒜臭。此药物疗效持久且毒性较低。使用时，口服每次约50mg，每日三次；肌注则为每日40mg。

用途

呋喃硫胺是维生素B1的衍生物，也被称为长效维生素B1，适用于治疗维生素B1缺乏导致的脚气病或Wernicke脑病，并可用于维生素B1缺乏引起周围神经炎、消化不良等症状的辅助治疗。

药理作用

作为维生素类药物，呋喃硫胺在体内与焦磷酸结合生成活性硫胺。此物质参与糖代谢过程中的丙酮酸和α-酮戊二酸氧化脱羧反应，对糖类代谢至关重要。当缺乏时，会导致氧化受阻形成丙酮酸，并使乳酸堆积，影响机体能量供应。

症状表现

症状主要体现在神经与心血管系统：感觉神经及运动神经受影响，表现为感觉异常、神经痛、四肢无力、肌肉酸痛和萎缩等；心血管方面，则因血中丙酮和乳酸增多导致小动脉扩张、舒张压下降、心肌代谢失调，从而出现心悸、急促、胸闷、心脏肥大、肝肺充血及周围水肿等症状。消化道表现为食欲减退，进一步引发衰弱与体重下降。

药代动力学

呋喃硫胺迅速转化为活性硫胺，在体内不被硫胺酶分解，组织亲和力强，维持时间长，具有高效、长效且低毒的特性。

适应症

适用于维生素B1缺乏导致的脚气病或Wernicke脑病治疗，并可用于辅助治疗由维生素B1不足引起的周围神经炎及消化不良等症状。

制备方法

呋喃硫胺可通过将硫胺盐酸盐开环后与四氢呋喃甲基硫代硫酸钠缩合制得。
