青霉素类药

【作用机制】青霉素类药物通过抑制细菌细胞壁合成,促使产生并活化自溶酶,导致细菌死亡。不同品种的药物针对不同的青霉素结合蛋白和酶,这些蛋白和酶参与了细菌细胞壁的代谢过程。

【适应证】这类药物主要在细菌繁殖期发挥杀菌作用。它们对革兰阳性球菌如链球菌、肺炎球菌以及敏感的葡萄球菌有较强的抗菌效果;同时,也对某些革兰阴性球菌如脑膜炎奈瑟菌和淋病奈瑟菌表现出良好的抗菌作用。此外,青霉素类药物还适用于白喉杆菌、梅毒螺旋体、回归热螺旋体、钩端螺旋体以及梭状芽孢杆菌(包括破伤风杆菌、气性坏疽杆菌和放线菌)引起的感染病症,如化脓性皮肤病、皮肤细菌感染、淋病、梅毒等。

【禁忌证】对青霉素过敏或有过敏史的患者禁用。

【不良反应】

  1. 过敏反应;
  2. 胃肠道不适。

【注意事项】

  1. 丙磺舒能阻滞青霉素类药物的排泄,联合使用时会使其血药浓度上升。
  2. 使用前必须按规范方法进行皮试;对普鲁卡因过敏者禁用普鲁卡因青霉素。
  3. 普鲁卡因青霉素偶可引发一种特异反应。注射后患者可能出现心里不适、濒危恐惧感、头晕、心悸等症状,一般无呼吸和循环障碍,多数情况下血压会升高。通常无需特殊处理,症状会在1~2小时内自行缓解;使用镇静药或抗组胺药有助于恢复。
  4. 低剂量青霉素不会引起毒性反应。若大剂量应用,则可能出现神经-精神症状。对于有血凝缺陷的患者,大剂量青霉素可能会扰乱凝血机制,从而导致出血倾向。
  5. 青霉素钠盐和钾盐水溶液不稳定,应现配现用。
