Naldemedine主要通过CYP3A酶代谢为去甲纳尔德梅丁【FDA标签】。部分代谢为纳尔德梅丁-3-葡萄糖醛酸酯是通过UGT1A3。这两种代谢物都是活性的,但效力低于纳尔德梅丁。这些代谢物的相对暴露量分别为9-13%和<3%,分别对应去甲纳尔德梅丁和纳尔德梅丁-3-葡萄糖醛酸酯。纳尔德梅丁在小肠中也会裂解,形成苯甲脒和纳尔德梅丁羧酸。
Naldemedine is mainly metabolized to nor-naldemedine by CYP3A [FDA Label]. Some metabolism to naldemedine-3-glucuronide occurs via UGT1A3. Both metabolites are acitive but less potent than naldemedine. The relative exposures of these metabolites are 9-13% and <3% for nor-naldemedine and naldemedine-3-glucuronide respectively. Naldemedine is also cleaved in the intestine to form benzamidine and naldemedine carboxylic acid.
