消化系统疾病种类繁多,用药复杂多样。对于反流性食管炎等食管疾病,主要应用制酸药物,如甲氰咪胍(泰胃美)等H2受体阻滞剂,它们能中和胃酸并抑制胃酸分泌,减少对食管的刺激。近年来,研究发现多潘立酮(吗丁啉)能增强食管下括约肌(LES)张力,这使得治疗效果有了显著提升。
消化性溃疡、慢性胃炎是常见的消化系统疾病。虽然至今尚未有特效疗法,但制酸药物在中和胃酸方面仍被广泛应用。过去常用的碳酸氢钠、钙铋镁等已逐渐被淘汰,而氢氧化铝因其饭后1-2小时服用可长达3小时中和胃酸的优点而广泛使用。与抗胆碱能药联合应用不仅可以缓解痉挛性疼痛,还能抑制晚间胃酸分泌,对消化性溃疡的治疗效果显著提高。
自20世纪50年代起,普鲁苯辛(溴化丙胺太林)作为常用的抗胆碱能药物被广泛应用。进入70年代后,H2受体阻滞剂如雷尼替丁、法莫替丁等相继问世,其抗酸作用更强且副作用减少,临床应用效果显著提高溃疡愈合率和缓解症状。
对于慢性胃炎的治疗,尽管目前尚不完全令人满意,但对胆汁反流性胃炎使用氢氧化铝、硫糖铝吸附胆盐,保护胃黏膜屏障,并配合多潘立酮(吗丁啉)治疗具有较好效果。研究发现十二指肠球部溃疡常合并胃窦炎,其中80%以上是由幽门螺旋杆菌(HP)引起的炎症病变。因此,在治疗时应同时使用相应的抗生素以促进溃疡愈合、减少复发及消除炎症。
此外,胶体铋制剂如次枸橼酸胶体铋(得乐),既能刺激黏液分泌保护胃黏膜屏障,又有一定的杀灭幽门螺旋杆菌作用。合理选择和应用上述药物对于提高消化性溃疡和慢性胃炎的治疗效果及改善预后具有重要作用。
胃蛋白酶等助消化药虽然现知疗效有限,但在某些情况下对慢性胰腺炎引起的蛋白质、淀粉和脂肪消化不良仍有应用价值。通常需要较大剂量才能见效。常用的缓泻剂包括镁乳、酚酞以及液状石蜡;而阿片类止泻药物在国内较少使用,常见的有氢氧化铝凝胶和苯乙哌啶等。
值得注意的是,苯乙哌啶具有类似吗啡的作用,易导致成瘾性,不宜长期应用。乳果糖是一种特殊双糖,在大肠内被细菌分解为乳糖和醋酸产生渗透性泻作用。酸化后的粪便能抑制肠道菌群产氨,并降低血氨水平,对于肝昏迷患者有一定治疗效果。
