止吐催吐药

呕吐是一种复杂的反射活动,各种止吐药可通过影响呕吐反射的不同环节而发挥止吐作用。这些机制包括抑制化学感受器、大脑皮质、耳前庭器官或直接作用于呕吐中枢。因此,在选择用药时应根据具体情况加以考虑。

通常情况下,预防性使用止吐药物的效果优于治疗性使用,尤其是在处理晕动症、放射病或化疗引起的呕吐时更为明显。对于妊娠呕吐,则需谨慎选用止吐药,一般仅推荐苯海拉明和茶苯海拉明等药物,在剧烈呕吐时才可考虑使用吩噻嗪类。

常用的止吐药物包括:

  1. 吩噻嗪类:如氯丙嗪和硫乙拉嗪(吐来抗)。
  2. 抗组胺药:例如苯海拉明、茶苯海拉明。
  3. 其他药物:东莨菪碱、甲氧氯普胺(胃复安,见“二十九、助消化药、消胀及胃肠运动功能调节药”),以及5-HT3受体阻滞剂等。

催吐药主要用于促使急性中毒患者将胃中的毒物呕吐出来。其催吐作用主要通过兴奋中枢催吐化学感受器或刺激消化道反射性地兴奋呕吐中枢来实现,如使用阿扑吗啡。然而,目前更常采用洗胃法替代催吐药物的使用。
