    Particulate air pollution and hospital admissions in Christchurch, New Zealand

    Aims: Winter air pollution in Christchurch is dominated by particulate matter from solid fuel domestic heating. The aim of the study was to explore the relationship between particulate air pollution and admissions to hospital with cardio-respiratory illnesses.Methods: Particulate air pollution statistics (PM10) were obtained from the Canterbury Regional Council monitoring station in the city. The New Zealand Health Information Service provided data on admissions to the Princess Margaret and Christchurch Hospitals for the period June 1988 through December 1998 for both adults and children with cardiac and respiratory disorders. The relationship between PM10 and admissions was explored using a time series analysis approach controlling for weather variables. Missing values were interpolated from carbon monoxide data for the same time period, as carbon monoxide and PM10 were highly correlated.Results: There was a significant association between PM10 levels and cardio-respiratory admissions. For all age groups combined there was a 3.37% increase in respiratory admissions for each interquartile rise in PM10 (interquartile value 14.8 mcg/m(3)). There was also a 1.26% rise in cardiac admissions for each interquartile rise in PM10. There was no relationship between PM10 and admissions for appendicitis, the control condition selected.Conclusions: In keeping with overseas studies, there is evidence in Christchurch of a relationship between ambient particulate levels and admissions with cardiac and respiratory illnesses. The size of the effect is consistent with overseas data, with the greatest impact for respiratory disorders.Implications: These results indicate that measures to control ambient particulate levels have the potential to reduce hospital admissions for cardio-respiratory illnesses.

    A medical device ( 110 ) including a catheter shaft ( 111 ) and a unitarily and continuously formed portion ( 108 ) having a varying durometer, and optionally including an expandable balloon ( 18, 118 ). One or both of the unitarily and continuously formed portion ( 108 ) and the balloon ( 18, 118 ) are made from an irradiation cross-linked or cross-linkable mixture of a polyamide elastomer and at least one additional cross-linking reactant. The polyamide elastomer can be a polyester amide, a polyether ester amide or a polyether amide, and is preferably a nylon block copolymer. The aromatic molecule can be 1,3,5 triethyl benzene; 1,2,4 triethyl benzene; and 1,3,5 triisopropyl benzene. The cross-linking reactant can be: (a) a difunctional material, (b) a trifunctional material, (c) a tetrafunctional material, or (d) an aromatic molecule containing at least two ring substituents, each of the ring substituents having labile hydrogens at a benzylic site therein. The cross-linking reactant can also be diallyl phthalate or meta-phenylene dimaleimide.

    A medical device ( 10 ) includes a catheter shaft ( 11 ) including inner and outer catheter shafts ( 12 and 14 ), and an expandable balloon ( 18 ) carried by the catheter shaft ( 11 ). The balloon ( 18 ) is made from an irradiation cross-linked mixture of a polyamide elastomer and at least one additional cross-linking reactant. The polyamide elastomer can be a polyester amide, a polyether ester amide or a polyether amide, and is preferably a nylon block copolymer. The cross-linking reactant can be: (a) a difunctional material, (b) a trifunctional material, (c) a tetrafunctional material, or (d) an aromatic molecule containing at least two ring substituents, each of the ring substituents having labile hydrogens at a benzylic site therein. The cross-linking reactant can also be diallyl phthalate or meta-phenylene dimaleimide. Irradiation is carried out by exposure to an electron beam or to ultraviolet, X- or gamma radiation, preferably at a total fluence of about 0.5 to about 20 megarads. The amount of the cross-linking reactant is selected to avoid the formation of gelling during the process by which the balloon ( 18 ) is made, and the amount of the cross-linking agent and the irradiation fluence are selected to give the balloon a strength generally about equal to that which would be obtained by mere irradiation.

    Medical dilatation balloons comprise a polymer that has the attribute of memory, and/or is crosslinked to impart memory. Such balloons exhibit a reduced tendency to overinflate at high inflation pressures. Furthermore, such balloons when shrunk radially by the application of heat while restraining axial shrinkage, exhibit customizable linear or non-linear compliance curves and lower crosslinking profile relative to the same balloon when unshrunk. Also disclosed is an expansive element within a tube whose outer diameter is equal to the outer diameter of the tube from which it was made. In addition, disclosed are (a) processes for preparing crosslinkable polymers, (b) joining crosslinked balloons to catheter systems, (c) forming shrunk balloon elements, and (d) forming an expansive element within a tube whose outer diameter is equal to the outer diameter of the tube from which it was made.
