生殖过程是一个复杂而精细的生理流程,包括精子与卵子的形成、成熟、排卵、受精、着床以及胚胎发育等多个环节。通过阻断其中任何一个环节,都可以达到避孕或终止妊娠的目的。
男性节育药物目前,在我国研究较为成熟的仅有一种男性节育药物——棉酚。它是由棉花根、茎和种子中提取的一种黄色酚类物质。使用后会减少精子数量;随着用药时间的延长,精子数量可以降至零。经4000多例健康男子试用,其有效率高达99%以上。
女性避孕药物根据作用环节的不同,女性避孕药主要分为以下几类:
抑制排卵的避孕药:这是最常见的一类女性避孕药物,由不同类型的雌激素和孕激素按不同剂量组成。主要包括短效口服避孕药(如复方炔诺酮片、复方甲地孕酮片)和长效注射避孕药(如复方己酸孕酮)。这类药物可能会出现早孕反应、子宫不规则出血、闭经、乳汁减少及凝血功能亢进等不良反应。对急慢性肝病患者、糖尿病患者及有子宫肌瘤的妇女不宜使用。
抗着床避孕药:此类药物通过干扰子宫内膜正常发育和转化,使其不利于孕卵着床。我国常用的大剂量炔诺酮或甲地孕酮即属于此类药物。这类药物的一大优点是应用不受月经周期限制。
主要影响子宫和胎盘功能的避孕药:目前正处于研究阶段,尚未大规模使用。主要药物为前列腺素类物质。
