曲美他嗪(Trimetazidine,TMZ),商品名为万爽力,是一种哌嗪类衍生物,化学名为1-(2,3,4-三甲氧基苯甲基)哌嗪二盐酸盐。其主要作用为抗心绞痛药物,通过对抗肾上腺素、去甲肾上腺素及加压素的作用,直接松弛血管平滑肌,降低血管阻力,增加冠脉和外周循环血流量,促进心肌代谢和能量产生。它还能减少心肌耗氧量,改善心肌氧供需平衡,并能增强对强心苷的耐受性,提高心绞痛患者的运动试验耐量,显著减少心绞痛发作频率并降低硝酸甘油用量。临床主要用于治疗冠脉功能不全、心绞痛及陈旧性心肌梗死;伴有严重心功能不全者可与洋地黄合用。
作用机制曲美他嗪对心脏的直接细胞保护作用,通过保存缺血氧细胞内的能量代谢,防止细胞内ATP水平下降,在维持细胞内环境稳定的同时确保离子泵的功能完善和跨膜钠-钾泵正常运转。它还能减少细胞内酸中毒、阻止心肌细胞内钠和钙的聚集,从而保护细胞收缩功能并限制氧自由基造成的细胞溶解和内膜损伤。
合成方法曲美他嗪的合成过程如下:首先将194克六水哌嗪溶于260毫升甲醇中,缓慢滴加60毫升甲酸甲酯。反应30分钟后加热回流5小时,冷却至室温后进行减压蒸馏,得到化合物甲酰哌嗪。随后将57克甲酰哌嗪与70毫升三甲氧基苯甲醛溶于反应瓶中,搅拌直至完全溶解。升温至100℃并缓慢滴加20毫升98%的甲酸,继续加热回流2小时。之后加入760毫升20%碳酸氢钠溶液,再进行回流2小时。冷却后用甲苯萃取三次,合并有机相水层,并调整pH值至5-6。静置后收集上清液,再次使用10%氢氧化钠溶液搅拌并静置,收集最终产物。无水硫酸铵干燥4小时后浓缩得到曲美他嗪。
曲美他嗪口服吸收迅速,单次20毫克服用后的血浆浓度峰值在1.8小时内达到53.6微克/升。长期服用(每天两次共15天)后,血浆浓度峰值可增加至84.8微克/升。其单剂量20毫克服用的药效-时间曲线下面积为79.3小时·微克/升,而每天两剂时则上升到260.6小时·微克/升。单独使用或与其他抗心肌缺血药物合用可降低心绞痛发作次数和硝酸甘油用量。
药效学曲美他嗪具有明显的药理作用,可用于治疗冠状动脉功能不全、心绞痛及陈旧性心肌梗死。在轻度心绞痛患者中,单独使用效果良好;而在中至重度心绞痛患者中,可作为辅助药物与其他抗心肌缺血药物联合应用。
临床应用对于伴有严重心功能不全的心绞痛患者,曲美他嗪可以与洋地黄类药物联用。在轻度心绞痛患者中,单独使用效果显著;而对于中至重度心绞痛患者,则可作为辅助治疗与其他抗心肌缺血药物联合使用。
此外,在普萘洛尔或β受体阻滞剂疗效不佳时,联用曲美他嗪可以有效减少心绞痛发作次数和降低硝酸甘油用量。
不良反应少数患者可能出现胃肠道不适症状(如恶心、呕吐)。
