抗疟药是指能杀灭或抑制疟原虫繁殖的药物,用于防治疟疾。早在公元一、二世纪,《神农本草经》已有记载常山具有抗疟作用。
最早的天然抗疟药物奎宁源自印地安人治疗热症时使用的金鸡纳树皮中的成分,也被称为金鸡纳霜。1907年,Rabe阐明了奎宁的化学结构。随后,人们积极进行合成抗疟药的研究。1926年,Schulemann合成了第一个人工抗疟药——扑疟母星(plasmochin),又称扑疟喹(pamaquin-e)。阿的平(atebrine)、氯喹(chloroquine)等相继问世。1944年,Woodward成功全合成奎宁并确认其结构。第二次世界大战期间,伯氨喹(primaquine)、戊喹(pentaquine)等取代了扑疟喹;通过生化代谢拮抗作用,合成了氯胍和乙胺嘧啶。
六十年代初,疟原虫对氯喹产生抗药性,掀起寻找新抗疟药的高潮。国外基于奎宁构效关系的研究,发展了喹啉甲醇类(quinoline-methanols)新型抗疟药,代表药物为甲氟喹(mefloquine)。我国则从4-氨基喹啉类药物的构效关系研究入手,合成了双咯喹(M6407)、咯啶与咯萘啶等具有双边四氢吡咯甲基氨酚侧链及其他结构的新抗疟药。这些新药不仅对心脏毒性低于氯喹,还具备不同程度的抗心律失常作用。
我国科学家从中草药中发现了常山(Radix dichroa)、甜茶(Hydrangea umbellata)、鸦胆子(Brucea javanica L. Merr.)等,并明确了常山碱的有效成分化学结构。七十年代初,又从黄花蒿,亦称青蒿中分离出青蒿素,经动物试验与临床观察证实其抗疟疗效优于氯喹,适用于抢救凶险型疟疾。青蒿素的化学结构研究确认它是一种含过氧桥的倍半萜,是抗疟药中的新型类型。1982年,这种过氧桥的倍半萜内酯完成了全合成。我国学者还从鹰爪植物中发现了另一种含过氧桥倍半萜的抗疟成分——鹰爪甲素。青蒿素及其衍生物还有显著的抗血吸虫作用。
此外,林可霉素类和四环素类抗生素也具有一定的抗疟作用,但由于作用较慢,通常不单独使用;而氯林可霉素与奎宁合用可用于治疗恶性疟疾。
